VENDOR CERTIFICATION

A vendor is a person or business that works with people who have physical or mental disabilities
to prepare for, gain or retain employment by providing medical services, training opportunities and/or

How do I become a vendor?

To apply for certification, please complete the following:
  1. Register with the My Florida MarketPlace (MFMP) statewide electronic procurement system at: vendor.myfloridamarketplace.com/. This online registration must take place prior to DVR’s Standard
    Vendor Application process being finalized.

  2. Locate your profession in the Vendor Qualifications Manual (PDF). Outlined under your occupation
    are the necessary qualifications and documents needed for submission to DVR to obtain certification.

  3. Complete the appropriate application (below) and Mail or Fax to (850) 245-3394
